Chronix Scribe records only what was said. If a drafted sentence about a diagnosis, medicine, dose or plan cannot be traced back to the transcript, it is removed and a small chip takes its place:
Left out: not said in the visit. Add it or leave it out.
It means the AI would otherwise have added something nobody said. Complete that part yourself, or delete the marker, before you sign. Sign-off asks you to confirm that you have reviewed every removal marker left in the note.
