Ambient clinical notes, drafted from what was said
Chronix Scribe listens to the consultation with the patient’s consent, transcribes it, and drafts a structured note where every clinical sentence links back to what was said. You review it, edit it and sign it. Audio is never stored.
Subjective
Right knee pain for six weeks, worse on stairs and after sitting.[00:48]No locking or giving way.[01:32]
Plan
Clinician said: continue physiotherapy, review in four weeks.[05:10]
Built for the consultation room
Ambient capture
Talk to the patient, not the screen. The conversation is transcribed as you talk.
Linked to what was said
Anything not said in the consultation is removed and marked.
Templates per specialty
Starter templates for each specialty, plus letters and forms. Sections with nothing stated stay empty until you fill them.
Patient summaries
A plain-language summary of what was agreed, written from the signed note. Translations are labelled as machine-translated so the patient knows to check.
Codes suggested for review
SNOMED CT codes are suggested from what the note documents and labelled “AI-suggested codes: review and confirm”. You confirm each one.
Team threads and 3D anatomy
Ask a colleague to look at a draft, or explain the anatomy with a 3D scene for the patient (optional add-on).
Four steps, with the clinician in charge at every one

Consent
Nothing captures until the patient has agreed. The consent script is shown on screen and the confirmation is recorded with the time.
Ambient capture
Speech becomes text as you talk. Audio is never stored.
A draft you can check
The draft records only what was said, with a timestamp on each clinical sentence. It never proposes a condition, a medicine or a plan of its own.
Review and sign
You edit, confirm any suggested codes and sign. The note is a draft until then, and a signed note cannot be changed, only added to.
Your patients’ data
Speech is transcribed live by Speechmatics in Europe and the audio is gone once it has been transcribed. No recording is kept, on any plan.
Notes and transcripts are encrypted and kept separate for each clinic. To draft a note, the text of the transcript, never the audio, goes to Anthropic in the United States.
Every draft carries the label “AI-generated: review before signing”. Only your signature turns it into a note.
One scribe, a way of working for each specialty
What Chronix Scribe is and is not
A documentation support tool for registered clinicians. It drafts notes, letters and patient summaries from what was said, links each clinical statement to its source, and leaves every note as a draft until you sign it. It does not make a diagnosis, triage, recommend treatment or act on its own.
Works alongside your record system
Copy a signed note, letter or summary straight into the record system you already use, or export it as a FHIR R4 document or JSON. Direct connections to some practice systems are listed in the Help centre.
