Supervision and countersigning

1 min read · Updated 29 September 2026

A supervisor can add a teaching note, a guideline, a required review or a countersignature to a colleague's note.

  • Supervision never replaces the clinician's own sign-off: the clinician still signs the note and remains responsible for it.
  • A countersignature is an additional attestation, recorded with its date, on top of the clinician's signature.
  • The clinician is notified when supervision is added to their note.
